LOS PASITOS EARLY INTERVENTION/ CHILD CARE CENTER
Tobosa’s adult services are focused on several principles fundamental to
quality of life. Individuals are encouraged to exercise choice and take
ownership of their lives. They are supported to actively participate in
all aspects of daily living, with the assistance of communication or
environmental control devices if needed. The adults served by Tobosa are
assisted to expand on their strengths and interests and develop meaningful
and valued social roles. They become contributing members of their
community who are appreciated and strengthened in their relationships
with others.

COMMUNITY LIVING

Community Living Services provide people with developmental
disabilities staff support in a residential setting in order to increase
or maintain community living skills, including self-advocacy,
communication, self-help, socialization, and leisure/recreation.
The residential program
provides adults with developmental disabilities with 24-hour assistance
and support in their homes. A person can live alone or with up to
three housemates, in spacious,
well-maintained homes located in safe, clean neighborhoods. Each home
reflects the preferences and tastes of each person, and ensures that
everyone can enjoy the privacy of their own room. Residential support
staff provide opportunities for active participation in daily routines and
preferred activities at home, in their neighborhood, and in their larger
community.
Assisted Living Services are provided to
people who do not require 24-hour support in the home. In assisted
living, people typically live alone or with a significant other.
Tobosa has also assisted a few couples in purchasing their own homes.

Family Living Services are also
offered by Tobosa and provided by a companion, foster or natural family member
who meets specific requirements. The person is supported in his or
her home or in the home of the provider.
Community living services are designed to assist people in utilizing
community resources and developing social connections. Many people in
the residential program are regular participants in city runs/walks and
frequent attendees at community activities, such as athletic and cultural
events as well as musical performances.

EMPLOYMENT back to top
The
supported employment program provides support for people who have a
desire to work. The ultimate goal of supported employment is to obtain
people community placements suited to each person’s strengths and
interests. Services in this component include career exploration, career
enhancement, job development, job placement, on-the-job training and
support, and job coaching.
Self Employment Services give
each person the opportunity, through a process of discovery, to start
his/her own business. Tobosa provides the necessary assistance to
develope a simple business plan, conduct a market analysis, as well as
all other bookkeeping duties necessary to maintain a small business.
Tobosa also supports the entrepreneur with community networking to
establish and maintain a successful business.
Individual
Supported Employment
is provided to people holding individual job placements in the community.
people are supported to become independent in all aspects of their
career, including work etiquette, transportation, building natural
supports on the job, and developing relationships with co-workers and
supervisors.
Supported employment services are intended to facilitate equal access to
valued employment and meaningful work for people with developmental
disabilities. Tobosa has supported many people in obtaining their
first-ever jobs in the community. As a result, these people now
enjoy relationships with non-disabled peers and are contributing members
of their community.

DAY HABILITATION

Day habilitation provides individualized activities that
support each person’s definition of a meaningful day.
There is a daily program
of functional and meaningful activities chosen by the person that relates
to his or her goals, objectives, interests and skills. Services and
supports are also designed to assist people to develop skills and relationships
that may lead to an eventual reduction of dependence on specialized staff
and supports. Each person is educated on their rights as citizens and the
role they play as responsible community members. |

COMMUNITY
ACCESS

Community
access services are
specifically intended to address each person’s independence, long-term
vision and personal goals. They are designed to promote maximum
participation in community life. These services are typically provided
individually or with two people. Tobosa utilizes local programs that
provide activities of personal interest, such as taking classes offered
in the community, maintaining memberships with service clubs and
organizations as well as volunteering their time at various community
locations.
Day habilitation/community access staff help acquaint community members
with the unique personality, strengths and gifts of each person that
establishes a foundation for relationship-building. The program has also
helped many people to develop valued social roles, such as a
Republican Party Campaign Committee member, musical performer, Salvation
Army volunteer, Pinatafest Committee member, Kiwanis Club member, and registered voter.

RESPITE

Respite services are available to families caring for children or adults
with developmental disabilities in their home. Respite is intended to
provide temporary relief to family members from their caregiving duties.
To be eligible for Tobosa’s
adult services, a person must qualify for
the Developmental Disabilities Waiver. To register for the Waiver, contact
the Income Support Division at (575) 625-3000. General information about
services available to people with developmental disabilities may be
obtained from Tobosa or the New Mexico Department of Health Regional
Office at (575) 624-6100.
